Atlantic Health’s Atlantic Rehabilitation outpatient team in Mountain Lakes, NJ, seeks a full-time Physical Therapist to deliver patient-centered care. The role emphasizes collaboration with a multidisciplinary team and ongoing professional development, with schedule shifts M/W/F 8am-4pm and T/TH 10am-6pm.

You will lead patient evaluations, develop plans of care, and provide education to patients and families, while integrating care across settings within a premier health system.

Qualifications

• New Jersey License to practice Physical Therapy.
• Graduate of an accredited Physical Therapy academic program.

Responsibilities

• Conduct comprehensive evaluations and develop patient-centered treatment plans.
• Collaborate with physicians, case managers, and rehabilitation team members.
• Communicate with patients and families to ensure understanding and continuity of care.
• Support various appointment types and adapt care plans as needed.
