Responsibilities include evaluating patients, creating and adjusting treatment plans, collaborating with physicians and rehab staff, and educating patients and families to ensure continuity of care.

Qualifications

• NJ license to practice Physical Therapy.
• Graduate of an accredited Physical Therapy program.

Responsibilities

• Conduct comprehensive evaluations and develop patient-centered treatment plans.
• Collaborate with physicians, case managers, and rehab team members.
• Communicate with patients and families to ensure understanding and continuity of care.
• Support various appointment types and adapt care plans as needed.
• Essential functions include patient evaluation, establishing plans of care and treatment programs, providing interventions, providing patient and family education, documenting interventions, and billing appropriately.
