Job summary

The Christ Hospital is seeking a licensed Physical Therapist to evaluate referred patients and plan evidence-based treatment programs to restore function, reduce pain, and maximize independence. You will document assessments, develop goal-driven plans, and coordinate with physicians for timely follow-up.

You will reassess progress, adjust plans as needed, and educate patients and families on home programs, while ensuring compliance with department policies and Medicare requirements.

Qualifications

• Must be licensed to practice Physical Therapy in Ohio.
• Experience preferred in outpatient setting.
• Excellent communication and patient education skills.

Responsibilities

• Evaluates patients and develops individualized treatment plans with measurable goals.
• Reassesses patients regularly with objective measures and updates goals.
• Implements treatment plans with supervision of staff and interns; ensures safety and documentation.
• Completes patient documentation and progress notes in a timely manner.
• Counsels patients and families on results, home exercises, and follow-up.
• Maintains department productivity and adheres to policies and quality standards.
