Qualifications

• Graduate from an accredited Physical Therapy program with Illinois licensure or eligibility.
• Strong patient-care and communication skills.
• Ability to lift >25 pounds and work in a fast-paced outpatient setting.
• Proficiency with documentation and use of EMR.

Responsibilities

• Provide exceptional outpatient rehabilitation services in a collaborative care model.
• Develop and adjust treatment plans and goals with physician input.
• Document patient status per compliance guidelines.
• Supervise rehab technicians and ensure timely charting.
• Maintain professional licensure and participate in continuing education.
