H2 Health in Dublin, VA is seeking a Physical Therapist to join our outpatient team. You will evaluate patients, develop care plans, and deliver evidence-based treatments to restore mobility and function.

Qualifications

• DPT, MSPT or BPT from a CAPTE-accredited program.
• Current Virginia PT license or eligibility.
• CPR certification or willingness to obtain.
• Willingness to travel between clinic locations.
• New graduates encouraged to apply.

Responsabilités

• Perform comprehensive patient evaluations and develop care plans.
• Deliver evidence-based treatments to improve mobility, strength and function.
• Treat orthopedic, post-operative, musculoskeletal, balance and vestibular conditions.
• Collaborate with patients, families, physicians and team members to reach outcomes.
• Educate on injury prevention, wellness and home exercise programs.
• Maintain accurate documentation of progress and outcomes.
• Utilize Scribe AI for streamlined documentation and time efficiency.
• Use Raintree EMR for documentation, scheduling and patient management.
• Manage a caseload of roughly 10–12 visits per day in outpatient setting.
• Contribute to a positive clinic culture focused on patient-centered care.
