Job summary

H2Health seeks a licensed Physical Therapist to deliver evidence-based outpatient care to diverse patient populations. You will evaluate patients, create and implement individualized plans, and use exercise, manual therapy, and modalities to improve mobility and quality of life.

You will collaborate with an interdisciplinary team, document progress, educate patients and families, and contribute to program development.

Qualifications

• Licensed physical therapist currently in good standing.
• Experience in outpatient settings preferred.
• Demonstrated commitment to evidence-based practice.

Responsibilities

• Provide one-on-one outpatient physical therapy care.
• Evaluate patients and develop individualized treatment plans.
