Job summary

P4 Physical Therapy is seeking a dedicated Physical Therapist to provide coverage in our outpatient clinic. You will assess patients, develop and initiate treatment plans, and document progress in a supportive team environment.

Qualified candidates are graduates of CAPTE-accredited programs, hold a state license, and possess CPR certification. Experience in outpatient orthopedic care is preferred. We offer a collaborative culture and opportunities for growth.

Qualifications

• Graduate from CAPTE-accredited Physical Therapist program.
• Current state license or ability to begin licensing promptly.
• CPR certification.
• Outpatient orthopedic experience preferred but not required.
• Excellent communication with patients, staff, and other healthcare professionals.
• Strong multi-tasking abilities to manage the clinic effectively.
• Self-starter with a mindset for growth and success.
• Passion for quality patient care with a compassionate approach.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluation results.
• Review the physician's referral and patient records to determine required PT treatment.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Oversee patient care performed by assistants and rehabilitation technicians.
• Complete a written plan of care, set goals, and develop a comprehensive program to attain them.
• Implement PT treatment programs and provide essential therapies.
• Communicate with patients/caregivers and colleagues, including physicians and insurance companies.
• Ensure timely documentation: daily notes, reports, evaluations, discharge summaries, and incident reports.
