Job summary

P4 Physical Therapy is seeking a dedicated Physical Therapist to provide coverage in our outpatient clinic. You will assess patients, develop and implement treatment plans, and collaborate with physicians and staff to support positive outcomes.

We value compassionate care, professional growth, and a supportive team environment. Eligible candidates should be CAPTE-accredited PT program graduates, licensed or eligible for licensure, CPR certified, and have outpatient orthopedic experience.

Qualifications

• Graduate from a CAPTE-accredited Physical Therapist program.
• Current state license or eligible for licensure promptly.
• CPR certification.
• Outpatient orthopedic experience preferred but not required.
• Excellent communication with patients, staff and physicians.

Responsibilities

• Assess patients to develop and initiate treatment plans.
• Review referrals and medical records to determine PT treatment.
• Provide direct patient care per established protocols.
• Document findings, progress, and patient instructions.
• Coordinate care with assistants and rehabilitation technicians.
• Develop a plan of care and a comprehensive program to reach goals.
• Implement treatment programs and essential PT interventions.
• Communicate with patients, caregivers, and healthcare professionals.
• Ensure timely documentation including notes and reports.
