Job summary

P4 Physical Therapy is seeking a dedicated Physical Therapist to provide outpatient clinic coverage. You will assess patients, develop plans, and deliver care following established guidelines in a collaborative, patient-centered environment.

You will work with physicians and staff, document progress, manage plan of care, and participate in ongoing professional development and team initiatives to advance quality care.

Qualifications

• Graduate from a CAPTE-accredited Physical Therapist program.
• Current state license or ability to obtain promptly.
• CPR certification.
• Outpatient orthopedic experience preferred.
• Excellent communication and teamwork.
• Self-starter with growth mindset and compassionate care.

Responsibilities

• Assess patients and develop treatment plans based on evaluation results.
• Review physician referral and patient records to determine PT treatment.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Coordinate patient care by overseeing assistants and rehabilitation technicians.
• Complete a written plan of care and develop goals and programs.
• Implement therapy and essential physical therapy treatments.
• Communicate with patients, caregivers, physicians, and insurers.
• Ensure timely documentation, including daily notes and reports.
