Job summary

Peak Performance Physical Therapy is seeking a full-time Physical Therapist to join our Brusly/Port Allen clinic in Louisiana. You will deliver patient-centered care, develop treatment plans, and work within a collaborative clinician team to optimize outcomes.

You will assess patients, implement plans, document progress, and coordinate with physicians and insurers while maintaining accurate records and compliance in a fast-paced, patient-first environment.

Qualifications

• Graduate from CAPTE-accredited Physical Therapy program.
• A current state of Louisiana license or the ability to obtain, and CPR certification.
• Able to create and implement outpatient balance and vestibular rehabilitation-setting treatment plans.
• Ability to work efficiently/effectively in an autonomous environment.
• Possesses basic PC skills and knowledge, including but not limited to EMR systems, Database software, and MS Office.

Responsibilities

• Assesses patients and develops and initiates treatment plans.
• Review physician referral, patient condition, and medical records to determine treatment.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Oversee care performed by assistants and rehabilitation technicians.
• Complete a written plan of care and develop goals.
• Implement physical therapy treatment program and essential therapies.
• Communicate with patients, caregivers, physicians, and insurers.
• Ensure timely documentation including daily notes and progress reports.
