Job summary

Advocate Health seeks a licensed Physical Therapist for outpatient rehabilitation at the Mountain Island Lake location. The position is full-time (40 hours/week) with a day-shift schedule in a fast-paced clinical setting.

Responsibilities include patient evaluation, goal-oriented treatment planning, and collaboration with physicians, staff, and families. Licensure, BLS, and a bachelor’s degree in physical therapy are required.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Applicable state license for Physical Therapy required.
• BLS certification required per policy guidelines.

Responsibilities

• Evaluates patients and develops care plans with modalities and exercise programs.
• Gathers data from medical records and physician reports to inform the care plan.
• Establishes functional goals and provides evidenced-based interventions.
• Documents evaluations, progress notes, discharge notes, and related charges.
• Communicates evaluation results and progress to physicians and other clinical staff; coordinates treatment and discharge planning.
• Supervises PTA(s) and Rehab Tech/Aide as required for licensure.
