Job summary

H2 Health is seeking a licensed Physical Therapist for our Columbia, KY outpatient clinic. The PT will evaluate patients, develop plans of care, and deliver evidence-based interventions to improve mobility and function in a collaborative, patient-centered setting.

The role supports post-operative rehab, balance disorders, and musculoskeletal care with strong team collaboration, education for patients, and robust documentation practices.

Qualifications

• DPT degree from a CAPTE-accredited program.
• Current Kentucky Physical Therapist license or eligibility for licensure.
• CPR certification, where applicable.
• Strong communication and interpersonal skills.
• Commitment to providing high-quality, patient-centered care.
• New graduates are encouraged to apply.

Responsibilities

• Evaluate patients and develop individualized plans of care.
• Provide evidence-based physical therapy interventions to improve mobility, function, strength, and quality of life.
• Treat a variety of orthopedic and musculoskeletal conditions, including post-operative rehabilitation and movement dysfunctions.
• Collaborate with Physical Therapist Assistants and support team members to support strong patient outcomes.
• Educate patients and caregivers on treatment plans, home exercise programs, and injury prevention strategies.
• Accurately document patient evaluations, treatment progression, and outcomes.
• Build meaningful patient relationships while delivering compassionate, patient-centered care.
