H2 Health in Berea, KY is seeking a Physical Therapist to join our outpatient team with flexible schedules: Full-Time, Part-Time or PRN. Our clinic offers diverse programs and a collaborative, patient-centered culture focused on mobility, function, and return to activity.

The role involves evaluating patients, developing evidence-based plans of care, and providing therapeutic exercise, manual therapy, and education.

Qualifications

• DPT (or MSPT/MPT/BPT) from a CAPTE-accredited program, licensed or eligible in Kentucky.
• Strong communication and collaboration skills with patients and staff.
• Commitment to high-quality, patient-centered care, new grads welcome.

Responsibilities

• Evaluate patients and develop individualized, evidence-based therapy plans.
• Treat outpatient caseload focusing on mobility, strength, function, and return to activity.
• Provide therapeutic exercise, manual interventions, and functional training.
• Educate patients on movement, recovery, injury prevention, and home programs.
• Collaborate with clinicians and support staff to coordinate care.
• Track progress and adjust plans to achieve meaningful outcomes.
• Complete timely and accurate clinical documentation.
