Qualifications

• Bachelor’s, Masters or Doctorate in Physical Therapy from an accredited program.
• Current Illinois Physical Therapy license.
• CPR certification required.

Responsibilities

• Evaluates patient and gathers information to determine diagnosis and plan.
• Sets functional goals based on clinical exam and findings.
• Devise detailed treatment and care plans for patients.
• Provides exceptional patient care and service.
• Maintain positive, respectful relationships with clients and colleagues.
• Cultivate relationships with patients and the community.
• Communicate with referring physicians to ensure consistent care.
• Maintain patient charts, logs and checklists for compliance.
• Protect patient confidentiality per HIPAA and company policies.
• Communicate well in a professional setting.
• Perform additional duties as required.

Education

Bachelor’s, Masters or Doctorate Degree from an accredited Physical Therapy Program

Current State of Illinois Physical Therapy license
