Qualifications

• Bachelor’s, Masters or Doctorate Degree from an accredited Physical Therapy Program.
• Current State of Illinois Physical Therapy license (or willingness to apply).
• CPR Certified.
• Experience in Outpatient Orthopedics.

Responsibilities

• Evaluates patient and gathers information to determine accurate diagnoses and treatment plan.
• Establishes functional and objective goals based on clinical examination.
• Devise detailed treatment and care plans for patients.
• Provides exceptional patient care and service.
• Maintain patient charts, clinical logs and checklists for compliance and HIPAA.
• Communicates with referring physicians and other sources to ensure consistent care.
• Maintain confidentiality of medical records.
• Perform additional duties as required.
