Discovery at Home Corporate in Port Saint Lucie, FL is seeking a dedicated Physical Therapist to provide comprehensive patient assessments and establish treatment plans. You will work directly with clients to relieve pain and maximize their functional performance.

The ideal candidate will be a graduate of an accredited Physical Therapy school, licensed or registered in Florida, and knowledgeable about Medicare and Medicaid guidelines. A comprehensive benefits package is included, and flexible scheduling is available for full-time team members.

Qualifikationen

• Must be licensed or registered by the applicable State without restrictions.
• Must have a valid driver’s license and reliable transportation.
• Must have a current CPR certification.

Aufgaben

• Provides initial and ongoing comprehensive assessments of the client’s needs.
• Establishes and revises treatment plans in consultation with physicians.
• Supervises Physical Therapy Assistants and paraprofessionals.
• Completes and submits OASIS assessments and significant changes in condition.
