Job summary

Eagle Crest Rapid Recovery in Houston, TX is seeking a PRN or part-time Outpatient Physical Therapist to join our post-acute rehab team. You will deliver outpatient therapy in designated community settings, assess function, and help seniors improve mobility and fall prevention with evidence-based interventions.

Open to PTs at all experience levels with licensure; new grads are welcome. This role offers flexible scheduling, minimal home health paperwork, and a collaborative in-house therapy staff

Qualifications

• Physical Therapist license is required.
• Open to PTs at all experience levels; new grads welcome.
• Outpatient PT experience is a plus but not required.

Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
