Eagle Crest Rapid Recovery in Houston, TX is seeking a PRN/Part Time Outpatient Physical Therapist Assistant to join a post-acute care team. The role focuses on outpatient therapy delivered in our settings, with emphasis on mobility, fall prevention, and geriatrics as residents receive care and therapy within designated communities.

Responsibilities include screening, planning and delivering evidence-based interventions, and coordinating with care teams.

Qualifications

• PTA license is required.
• Open to PTAs at all experience levels; New Grads welcome.

Eagle Crest Rapid Recovery in Houston, TX, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated PRN or Part Time Outpatient Physical Therapist Assistant for our rehab program!

Outpatient therapy delivered where seniors live, not traditional home health. You'll build ongoing relationships with residents in a few designated communities, treat higher-functioning patients, avoid most of the paperwork associated with home health, and have flexibility in your schedule while making a significant impact on fall prevention, mobility, and wellness.

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Physical Therapist Assistant license is required.
