Responsibilities

• Evaluates patients and participates in rehabilitation planning.
• Plans and delivers outpatient PT treatments per standard of care.
• Coordinates care with clinical team for optimal patient outcomes.
• Educates patients/caregivers to promote home exercise programs.
• Completes documentation reflecting care provided and outcomes.
• Monitors and evaluates treatment outcomes.
• Maintains current professional knowledge and licensure.
• May supervise students after one year of experience.
