Job summary

Enhabit Home Health & Hospice is expanding its physical therapy team to deliver in-home care across the community. The PT will evaluate and treat individuals with mobility impairments, using modalities and therapeutic techniques to improve daily living activities and independence.

The role requires current state licensure as a Physical Therapist, reliable transportation, and the ability to travel to patient homes in varying conditions.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities to improve mobility and function.
• Provide skilled care to clients per physician orders and plan of care under supervision.
• Educate family and health care team members to support patient outcomes.
