Job summary

HonorHealth is seeking a Physical Therapist for its Therapy Services OP-Via de Ventura location in Scottsdale. The role focuses on evaluation, administration, and oversight of physical therapy services for individuals with physical disabilities, aimed at restoring function and enhancing daily living activities.

The therapist will work with an interdisciplinary team, develop individualized care plans, and ensure documentation and compliance with state and regulatory standards.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited university approved by the APTA.
• Masters in or Doctorate in Physical Therapy from an accredited university approved.

Responsibilities

• Implements an evidence-based physical therapy program for individuals with physical disabilities.
• Develops and updates plan of care in collaboration with patient and family.
• Collaborates with physicians, nurses, and other care team members.
• Maintains accurate documentation, including evaluations, treatment notes, and discharge summaries.
• Supervises therapy assistants and coordinates care with interdisciplinary teams.
