Telos Health Systems, Inc. in Jacksonville, FL seeks Outpatient Physical Therapist (PT) and Physical Therapy Assistant (PTA) on a PRN basis to support Sequoia Care across outpatient, long-term care, home health and assisted living settings.

PT hourly rate is $47–$55 and PTA hourly rate is $32–$40, plus a $250 monthly travel stipend for part-time and full-time staff. We offer flexible opportunities, a clinician-first culture and strong team support to grow professionally.
