Job summary

HonorHealth is seeking a Physical Therapist for its Network Float Pool in Arizona. The role focuses on evaluation, treatment, and oversight of evidence-based physical therapy services for individuals with functional limitations, in collaboration with patients and families.

Responsibilities include developing care plans, maintaining documentation, and working with an interdisciplinary team to optimize outcomes.

Qualifications

• Bachelor's in Physical Therapy required.
• Current AZ PT license in good standing required.
• BLS certification required.
• Experience in a network float pool or inpatient/outpatient setting preferred.

Responsibilities

• Implements individualized PT plans using evidence-based techniques and modalities.
• Evaluates patients and develops comprehensive, measurable plans of care with discharge goals.
• Maintains accurate, timely documentation and billing in the EMR system.
• Collaborates with the interdisciplinary team and supervises PT assistants as needed.
• Participates in departmental initiatives, staff meetings, and quality improvement.
