Job summary

Eagle Crest Rapid Recovery in Houston, TX is seeking a PRN/part-time Outpatient Physical Therapist to join our geriatric-focused rehab program. The ideal candidate will hold a Physical Therapist license and be comfortable treating high-functioning seniors in outpatient settings outside traditional home health.

• Stand, walk, and assist with lifting, transferring, and repositioning as patient care requires.
• Administer manual techniques, ambulation, therapeutic exercises, modalities, devices, and wound care.
• Provide consultation to patients, families, caregivers, and other providers.
• Generate discharge summaries ensuring regulatory compliance.

OUTPATIENT PHYSICAL THERAPIST – PRN/ PART TIME

EAGLE CREST RAPID RECOVERY – HOUSTON, TX

Eagle Crest Rapid Recovery in Houston, TX, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated PRN or Part Time Outpatient Physical Therapist for our rehab program!

Outpatient therapy delivered where seniors live, not traditional home health. You'll build ongoing relationships with residents in a few designated communities, treat higher-functioning patients, avoid most of the paperwork associated with home health, and have flexibility in your schedule while making a significant impact on fall prevention, mobility, and wellness.

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
