Job summary

Mason Creek Transitional Care in Katy, TX is seeking a PRN Physical Therapist to join our in-house rehab program. You will assess and treat patients with physical functioning disorders, participate in development of care plans, and help guide discharge planning to support recovery.

Applicants must hold a Physical Therapist license and may be at any experience level; new grads are welcome to apply. This position offers flexible scheduling within our post-acute care setting in Katy.

Qualifications

• Physical Therapist license is required.
• Open to Physical Therapists at all experience levels.

Responsibilities

• Conduct screening and evaluation of patients with physical functioning disorders and wound care, and develop care plans.
• Deliver rehabilitative, skilled and medically necessary treatment interventions per regulatory and clinical standards.
• Administer procedures as part of the rehabilitation plan including manual techniques, ambulation, exercises, modalities and wound care.
• Provide consultation and counseling to patients, families, caregivers and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries ensuring regulatory and clinical compliance.

Mason Creek Transitional Care in Katy, TX, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a PRN Physical Therapist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
