Job summary

BayCare Health System is seeking an Outpatient Physical Therapist at Bartow Medical Center in Bartow, FL. The role focuses on one-on-one, patient-centered outpatient therapy with evidence-based treatment plans and thorough documentation.

Requirements include a Bachelor’s in Physical Therapy, BLS certification, and a valid PT license or eligibility. Occasional weekend shifts and no remote work are expected.

Qualifications

• Current BLS certification is required.
• Licensed Physical Therapist or eligible to obtain licensure.
• Bachelor's degree in Physical Therapy is required.
• Temporary PT license with supervision allowed within 6 months.

Responsibilities

• Evaluate and develop treatment programs for individual patients in outpatient settings.
• Develop and implement evidence-based plans to improve functional outcomes.
• Provide one-on-one, patient-centered therapy.
• Maintain timely and accurate documentation per standards.
• Monitor patient progress and adjust plans as needed.
• Contribute to program development for neuro-focused services (vestibular/balance, gait, cognitive-motor).
• Supervise PT Assistants and Rehab staff under licensed PT supervision.

Physical Therapist Responsibilities

• Evaluates and develops specific treatment programs for individual patients/residents based on their needs and in accordance with the principles and practices of physical therapy.
• Develop and implement evidence-based treatment plans to improve functional outcomes.
• Provide one-on-one, patient-centered therapy in an outpatient setting.
• Maintain timely and accurate documentation in accordance with professional and regulatory standards.
• Monitor patient progress and adjust treatment plans as necessary.
• Contribute to program development for neuro-focused services, including vestibular or balance training, gait rehabilitation, or cognitive-motor integration.
• Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants, Rehabilitation Technicians, Massage Therapists, Athletic Trainers, Fitness Specialists, Physical Therapy and Physical Therapy Assistant students and volunteers.
• Team members with temporary permits must work under the direct supervision of a licensed Physical Therapist that has been licensed for a minimum of six months. A supervising licensed Physical Therapist may only supervise one permittee at any given time.

• Or PT (Physical Therapy) Eligible to obtain
• Or PT (Physical Therapy) Temporary and licensure within 6 months
• Required Bachelor's Physical Therapy
• Preferred 1 year Physical Therapist
