Job summary

UVM Health is seeking a Pharmacist to assure the appropriateness of drug therapy, ensure inpatient and outpatient medication needs are met, and maintain all required records. The role may require stepping into managerial duties in the absence of the Manager.

Qualified candidates hold a Bachelor of Science in Pharmacy; a PharmD is preferred. Vermont licensure is required. Residency or equivalent experience is preferred, with two years of clinical hospital pharmacy experience.

• ASHP-accredited Pharmacy Residency or equivalent experience preferred.
• Licensed Pharmacist in Vermont is required.

The Pharmacist assures the appropriateness of drug therapy as described in the position accountabilities. The pharmacist is responsible for ensuring that the medication needs of inpatients and outpatients are accurately met and for maintaining all necessary records. In the absence of a Manager, a Pharmacist may be required to assume the essential responsibilities and perform the duties of the Manager.

EDUCATION:

Bachelor of Science Degree from an ACPE accredited School of Pharmacy required. Doctor of Pharmacy Degree preferred. ASHP accredited Pharmacy Residency or equivalent experience preferred. Licensed Pharmacist in the State of Vermont

EXPERIENCE:

Two years clinical pharmacist experience in a health system preferred.
