Job summary

AdventHealth in Tavares, Florida is seeking a licensed Physical Therapist to evaluate and treat patients in our Sports Medicine and Rehab location. You will determine diagnoses, develop plans of care, coordinate referrals, and document progress to ensure high-quality patient outcomes.

Location: Sports Medicine and Rehab, 3140 Waterman Way, Tavares, FL 32778. Competitive pay range ($66,268.80–$123,281.60) with sign-on and relocation bonuses, full-time schedule, and benefits from day one.

Qualifications

• Doctorate in Physical Therapy (preferred)
• Licensure as a Physical Therapist or PT-TEMP (Required)
• BLS certification (Required)

Responsibilities

• Determines physical therapy diagnosis and establishes plan of care.
• Designs and implements treatment plan of care per standards and guidelines.
• Assesses patient mobility and evaluates musculoskeletal and functional status.
• Oversees PTA aides and participates in performance improvement initiatives.
• Documents and codes treatment accurately per policy.

Job Description:

• Up to $10,000 Sign-On Bonus for Eligible Applicants
• Up to $3000 Relocation Bonus for Eligible Applicants
• Schedule: Several shifts available Monday-Thursday 9am-6pm, Friday 8am-5pm or working 4 days a week for 10 hours

• The anticipated minimum pay rate for this position is $36/hour
• Determines physical therapy diagnosis and establishes a plan of care, making recommendations for referral if treatment is needed outside the scope of physical therapist practice.
• Designs and implements treatment plan of care utilizing practice standards, recommended techniques, modalities, insurance guidelines, and patient needs.
• Assesses patient ability to safely and effectively perform functional mobility skills and evaluates muscle, nerve, joint, and integumentary systems.
• Supports consumer marketing and public awareness through participation in community events, educational lectures/speaking, and/or other extension of clinical expertise to the community.
• Oversees the care provided by physical therapist assistants and rehabilitation therapy aides, according to established regulations and practice standards. Participates in practice changes as a result of performance improvement activities and supports quality standards and initiatives set by the department.
• Documents and charges accurately, completely, and timely using measurable and objective terminology following standards of practice, insurance guidelines and department/hospital policy and procedures.
• Participates in marketing to physicians and physician practices to build referral relationships. Performs evaluations and re‑evaluations using recommended techniques and standards.
• Monitors patient response and progress to treatment, modifying the treatment plan as appropriate.
