Job summary

Advocate Good Shepherd Hospital in Hoffman Estates, IL is seeking a full-time Occupational Therapist for Outpatient Rehabilitation: Fitness Center. The role involves 36 hours per week with two evenings until 7:00 p.m.

Experience in lymphedema is required; brain injury and ortho experience are preferred. A $5,000 sign-on bonus is available for new hires, with emphasis on comprehensive patient care and teamwork.

Qualifications

• Master’s Degree in Occupational Therapy from an accredited school or educational requirements commensurate with graduation date.
• Licensed Occupational Therapist in the state where you work; BLS for Healthcare Providers per American Heart Association.

Responsibilities

• Interprets physician referrals and evaluates patient functional, perceptual, cognitive, social and vocational abilities and impact on activities of daily living.
• Establishes a plan of care; provides and/or fabricates assistive devices and/or splints; monitors outcomes and adjusts plans as needed.
• Completes documentation, billing, scheduling, and discharge planning; communicates with physicians and team members.
