Job summary

Qualifications

• Certified Lymphedema therapist and outpatient rehabilitation experience required.
• Bachelor's degree in Physical Therapy and state licensure required.
• BLS certification required per policy guidelines.

Responsibilities

• Evaluates patients and establishes care plans including modalities and exercise programs.
• Gathers data from medical records and patient/family for documentation.
• Sets functional goals with patients and provides evidence-based interventions.
• Documents evaluations, tests, progress notes, and discharge details.
• Coordinates schedule and discharge planning with physicians and staff.
• Supervises PTA's and Rehab Tech/Aide as required for licensure.
