Job summary

Fairview Health Services is seeking an orthopedic physical therapist for outpatient clinics serving adolescents to adults across the Twin Cities metro. MDT training is preferred; mentoring is available to build proficiency.

The PT will conduct examinations, plan care, and collaborate with patients and families to achieve outcomes. Responsibilities include evaluation, developing goals, coordinating with the interdisciplinary team, and establishing discharge plans.

Qualifications

• Licensed Physical Therapist in Minnesota (or temporary license)
• Basic Life Support (American Heart Association or Red Cross)
• MDT training or willingness to pursue (mentoring provided)

Responsibilities

• Perform evaluation and re-evaluation with patient and family.
• Develop an individualized plan of care with goals.
• Coordinate care with interdisciplinary team for all aspects of patient care.
• Establish discharge plan and monitor progress.
• Educate patient and family to support learning needs and safety.
