Job summary

Fairview Health Services is seeking a dynamic orthopedic physical therapist to join our outpatient flexible workforce. The therapist will cover adolescent to adult patients across multiple clinics in the greater Twin Cities area, with MDT training preferred and mentoring offered as needed.

Responsibilities include examination, planning, and delivering patient-centered care, coordinating with families and the care team, and establishing outcomes.

Qualifications

• Minnesota Physical Therapy license (or temporary) required.
• BLS certification required.
• Doctorate of Physical Therapy preferred.
• At least 1 year of outpatient orthopedic therapy experience preferred.
• MDT experience and training preferred.

Responsibilities

• Perform Evaluation/Re-evaluation with patient and family.
• Gathers pertinent data and performs examination.
• Develops an individualized plan of care with patient and family.
• Coordinate care and communicate with interdisciplinary team.
• Establish discharge plan and patient education.
