Qualifications

• Current Physical Therapy license to practice in Minnesota or eligible for temporary licensure.
• Basic Life Support required (American Heart Association or Red Cross).

Responsibilities

• Perform evaluation and re-evaluation of patients.
• Develop and implement individualized plan of care with patient and family.
• Coordinate care with interdisciplinary team and communicate effectively.
• Establish discharge plan and measure outcomes.
