Endeavor Health is seeking an Occupational Therapist II for Outpatient Physical Rehab Services in Rolling Meadows, IL. This per diem role offers weekday daytime hours (8:45am–5:15pm) with no weekend shifts.

You will assess and treat patients, develop and implement individualized plans, document outcomes, and collaborate with physicians, nurses, and staff to ensure high-quality care. Illinois license or eligibility and NBCOT certification are required.

Qualifications

• Assess, treat, and care for patients in outpatient rehab per plan.
• Develop treatment plans based on patient development and goals.
• Maintain patient records, billing and documentation per guidelines.

Responsabilités

• Assess functional abilities, ROM, pain, and ADLs to guide treatment.
• Administer and instruct patients in treatment plans and monitor progress.
• Prepare and complete patient records and documentation.
• Collaborate with physicians, nurses, and staff to provide optimal patient care.
• Educate patients and families; support discharge planning.
• May supervise paraprofessionals; delegate tasks as needed.
• Adhere to safety and infection control procedures.
