Job summary

Endeavor Health is seeking a Physical Therapist II to join the outpatient rehabilitation team in Rolling Meadows, IL. This full-time role focuses on 1:1 patient care, development and execution of treatment plans, and close collaboration with physicians and staff to ensure high-quality care.

Responsibilities include evaluating function, documenting care, and educating patients and families on plans of care. Required degree in PT, IL license, and CPR certification; position includes growth

Qualifications

• Degree in Physical Therapy from an APTA-accredited program.
• Current Illinois licensure or eligibility.
• CPR certification through the American Heart Association.

Responsibilities

• Provide 1:1 patient care and avoid double booking.
• Assess, treat, and care for patients per developmental needs; set treatment goals.
• Evaluate functional abilities (strength, ROM, pain, balance) and plan modalities.
• Prepare and complete patient records, billing, and documentation per guidelines.
• Collaborate with physicians, nursing, and staff to ensure optimal care.
• Engage patients and families in care planning and education sessions.
