Job summary

Endeavor Health is seeking an Occupational Therapist II for Outpatient Physical Rehab Services in Rolling Meadows, IL. The role involves evaluating and treating patients, developing plans, and coordinating with a multidisciplinary team to restore function and independence.

Requires OT degree from an AOTA-accredited program, Illinois license or eligibility, NBCOT certification, and AHA CPR. Per diem hours are 8:45am–5:15pm, Monday–Friday, with no weekends.

Qualifications

• Degree in Occupational Therapy from an AOTA-accredited school or the equivalent.
• Current Illinois licensure or eligibility.
• Certification by National Board for Certification in Occupational Therapy (NBCOT).
• CPR certification from American Heart Association (AHA).

Responsibilities

• Assess, treat, and care for patients in outpatient rehab.
• Develop and implement treatment plans to restore function, increase strength, and reduce pain.
• Document patient records and billing per departmental guidelines.
• Collaborate with physicians, nursing and rehab staff to provide optimal care.
• Educate patients and families and plan post-discharge care.
