Qualifications

• 6 months previous RN experience
• Associates or Bachelor’s degree in Nursing preferred
• Current unencumbered NYS RN license
• Basic Life Support certification

Responsibilities

• Take vital signs and document in EMR
• Review and update patient medications in the EMR
• Complete referrals and track patient compliance
• Review prescriptions electronically and forward to providers
• Obtain patient consent for procedures
• Perform pre-visit planning and review quality metrics
• Educate patients on medication, testing procedures, and home care
• Maintain clean and safe work environment and cabinets
• Communicate effectively via EMR and care team
• Participate in daily patient care huddles and team tasks
