Qualifications

• Bachelor’s degree in Occupational Therapy is required.
• Licensed Occupational Therapist (OT) in Illinois is required.
• BLS certification within 30 days of hire is required.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed.
• Maintains safe body mechanics to reduce risk of injury to self, staff and patients.
• Designs an appropriate plan of care aligning goals, treatment, outcomes, and discharge plan.
• Adjusts care based on patient response and available evidence.
• Educates patients, family, caregivers, students, and other health care providers.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examinations using reliable methods related to chief complaint, screening, and history.
• Screens patients to determine effectiveness and need for further therapy services.
• Uses support personnel according to legal standards and ethical guidelines.
• Monitors care delivered by support personnel and provides feedback.
• Documents evaluations and plans of care including goals and status.
