Responsibilities

• Performs patient examinations and determines diagnoses and outcomes of care.
• Designs an individualized plan of care with goals and discharge planning.
• Educates patients, family, and caregivers using effective teaching methods.
• Documents evaluations and plans of care per policy.
• Supervises COTA and rehabilitation technicians as applicable.
• Communicates progress and discharge recommendations with the medical team.
