Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed.
• Maintains safe body mechanics to protect patients and staff.
• Designs and updates an individualized plan of care with goals and discharge plan.
• Adjusts care based on patient response and evidence.
• Educates patients, families, caregivers, students, and other clinicians.
• Evaluates clinical findings for PT diagnoses and outcomes.
• Documents evaluations and plans of care, including goals and status.
• Collaborates with patient and family and supervises COTA and rehab technicians.
• Communicates progress and discharge needs to the medical team.
