Qualificações

• Previous experience in addiction recovery or behavioral health settings.
• Familiarity with MAT including methadone, buprenorphine, and naltrexone.
• Experience conducting individual and group counseling sessions for recovery.
• Knowledge of CBT, MI, and relapse prevention techniques.
• Documentation of notes, treatment plans, and progress per guidelines.
• Familiarity with state regulatory standards (42 CFR Part 2, HIPAA, Joint Commission).
• Experience collaborating in a multidisciplinary team with medical staff and peers.

Responsabilidades

• Provide high quality, compassionate individual and group counseling.
• Plan, oversee, facilitate and document patient recovery.
• Co-facilitate group or family sessions as needed.
• Maintain timely, clear documentation of patient care and incidents.
• Prepare individual treatment plans for each patient.
• Conduct initial and follow-up assessments.
• Evaluate needs and coordinate referrals to other programs.
• Plan for aftercare for assigned patients.
• Provide crisis intervention when required.
• Perform case management to ensure quality care.
• Liaise between referral sources and patients as needed.
