Job summary

Advocate Health and Hospitals Corporation seeks an Occupational Therapist to assess patients with lymphedema and other conditions in outpatient settings. You will develop plans of care, fabricate devices as needed, and coordinate discharge planning with families and care teams.

Responsibilities include thorough documentation, adherence to billing/compliance policies, maintaining productivity, and ensuring high-quality rehabilitative care across the continuum.

Qualifications

• Master’s Degree in Occupational Therapy from an accredited program.
• Licensed Occupational Therapist in the state.
• Lymphedema work experience required.
• Outpatient OT experience preferred.
• Strong communication and organization skills.
• Proficient in documentation and billing processes.

Responsibilities

• Interprets referrals and evaluates patient abilities to determine impact on daily living.
• Establishes a plan of care and implements treatment; fabricates assistive devices as needed.
• Plans for discharge including home programs and caregiver instruction; coordinates with care team.
• Documents care and progress per policy; handles scheduling and billing considerations.
• Maintains safety and infection control measures and promotes patient safety.
