Responsibilities

• Complex Chronic Disease Management: diagnose and manage advanced chronic conditions.
• SDOH Intervention: screen for and address non-medical barriers to health.
• Trauma-Informed Care: use trauma-informed communication with patients under chronic stress.
• Interdisciplinary Care Coordination: collaborate with social workers, case managers, CHWs, and behavioral health specialists.
• Preventive Medicine & Harm Reduction: immunizations, routine screenings, harm reduction for high-risk behaviors.
• Resource Optimization: navigate financial barriers, use sliding scales and low-cost formulary meds, leverage public programs.
• EHR Documentation: maintain accurate documentation in the Epic EMR.
