Job summary

Trinity Health Mid-Atlantic is seeking a Board-Certified/Board-Eligible Internal Medicine Physician to support expanded primary care access in the Darby, PA area. This 100% outpatient role offers a team-based care model with on-site support and integrated Epic EMR.

You will diagnose and manage complex chronic diseases, address social determinants of health, and work with on-site social workers, case managers, and behavioral health specialists to provide holistic patient care.

Qualifications

• Board-certified or board-eligible in Internal Medicine.

Responsibilities

• Provide 100% outpatient internal medicine care.
• Manage complex chronic diseases (diabetes, hypertension, etc.).
• Address social determinants of health (SDOH) in patient care.
• Collaborate with interdisciplinary care team including social workers and care managers.
• Utilize Epic EMR for documentation and care coordination.
