Job summary

Trinity Health Mid-Atlantic's Primary Care – Darby is seeking a Board-Certified/Board-Eligible Family Medicine or Internal Medicine physician to support expansion and access initiatives across the Pennsylvania market.

The practice is 100% outpatient with a team-based care model, integrated Epic EMR, and strong referral networks. You will manage complex chronic diseases, address social determinants of health, and collaborate with on-site social workers and care teams to improve community health.

Qualifications

• Board-Certified/Board-Eligible Family Medicine or Internal Medicine physician.
• Experience with primary care expansion and access initiatives.
• Familiarity with ambulatory care and Epic EMR.

Responsibilities

• Diagnose and manage complex chronic diseases.
• Address Social Determinants of Health (SDOH) barriers to access.
• Practice trauma-informed care and patient-centered communication.
• Coordinate interdisciplinary care with on-site social workers, case managers, and behavioral health specialists.
• Provide preventive medicine and harm reduction measures.
• Navigate resources to optimize care costs and leverage community programs.
• Document care effectively in the Epic EMR.
