Job summary

Trinity Health Mid-Atlantic Medical Group is seeking a Board-Certified/Board-Eligible Internal Medicine physician for the Medical Arts Internal Medicine practice in Langhorne, PA. This 100% outpatient role offers immediate patient volume and a team-based care model with Epic EMR integration.

You will manage complex chronic diseases, address social determinants of health, and participate in trauma-informed care while coordinating with on-site social workers, case managers, and behavioral health

Qualifications

• MD or DO with board eligibility or certification required.
• Board certification/eligibility in Internal Medicine preferred.
• Experience in outpatient primary care is advantageous.

Responsibilities

• Provide 100% outpatient internal medicine care with no inpatient duties.
• Deliver comprehensive chronic disease management and preventive care.
• Collaborate with APPs and care team for coordinated patient care.
• Engage in social determinants of health interventions and trauma-informed communication.
• Utilize Epic EMR for documentation and care coordination.
