Job summary

Indiana Health Centers, Inc. (IHC) – South Bend Health Center is seeking an experienced NP/PA for outpatient primary care.

You will manage patients of all ages, perform exams, diagnose illnesses, and provide preventive care counseling in a team-based care model using the eClinicalWorks EMR. Responsibilities include collaborating with physicians, attending staff meetings, and contributing to programs under the Site Medical Director.

Qualifications

• Nurse Practitioner certification from ANCC or AANP required.
• Indiana license and current DEA licensure/prescriptive authority needed.
• Minimum 2+ years of NP/PA experience in primary care; CHC experience preferred.
• BLS certification required.
• Spanish speaking skills preferred but not required.

Responsibilities

• Perform physical examinations and establish patient history and data.
• Diagnose illnesses and develop management plans with supervising physicians.
• Provide counseling on preventive care, medications, and nutrition.
• Attend clinic staff meetings and participate in projects under the Site Medical Director.
• Follow HIPAA and OSHA standards for medical care providers.
