United Future is seeking a Family Nurse Practitioner to join our physician-led, Long Island-based practice. The role focuses on 100% outpatient care with no on-call requirements and flexible full-time or part-time hours.
The candidate will deliver preventive, acute, and chronic care, coordinate with specialists, and guide a growing patient panel in a supportive, HIPAA-compliant environment with strong clinical and administrative backing.
Qualificações
- BSN and MSN from an accredited nursing program.
- NYS RN and NP license, BLS, board certification.
- Experience and ability to grow a local clinical practice.
Responsabilidades
- Serve as a key clinical provider in delivering high-quality care.
- Provide direct patient care in outpatient primary care, including preventive, acute, and chronic disease management.
- Refer patients to specialized services as needed.
- Educate patients on diagnoses, treatment options, and preventive strategies.
- Maintain accurate EMR documentation.
- Collaborate with physicians, specialists, and care teams for seamless patient care.
- Provide clinical oversight to nurses, technologists, and support staff.
- Stay compliant with HIPAA, NYS DOH, CMS, and payer guidelines.
- Participate in population health initiatives and quality improvement.
- Contribute to developing a sustainable patient panel.
