Job summary

Fresenius Kidney Care is seeking a professional registered nurse at the Outpatient RN CAP 2 level for our End Stage Kidney Disease (ESKD) health care team. The role covers assessment, planning, intervention and evaluation for assigned patients, with potential delegation to RNs, LVN/LPNs and patient care technicians.

The RN CAP 2 will participate in education, quality improvement and may serve as a Preceptor, advancing through the Clinical Advancement Program per policy and state requirements.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR/BLS Certification.
• Must meet the practice requirements in the state in which he or she is employed.
• Entry level for RNs with minimum of 2 years of Nephrology Nursing experience.

Responsibilities

• Perform ongoing, systematic collection and analysis of patient data pre– during – post hemodialysis treatment and document in the patient record.
• Adjust treatment plans as indicated and notify supervisor or physician as needed.
• Educate patients and families about education needs and treatment options.
• Direct and provide safe and effective delivery of hemodialysis therapy with the care team.
• Serve as a resource, mentor and role model for staff and participate in quality improvement.
