Job summary

Fresenius Medical Care Outpatient RN CAP 1 is an entry-level role in the Clinical Advancement Program, accountable for assessment, planning, intervention, and evaluation of care for an assigned patient group undergoing hemodialysis.

Responsibilities include coordinating with the care team, documenting patient data, and contributing to quality improvement and patient education while meeting CAP requirements for advancement.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful CPR BLS Certification.
• Must meet the practice requirements in the state in which he or she is employed.

Responsibilities

• Assesses, plans, and evaluates care for assigned patients undergoing hemodialysis.
• Collaborates with RNs, LVN/LPNs and Patient Care Technicians to deliver safe dialysis therapy.
• Documents patient data and adjusts treatment plans as needed in the medical record.
• Provides patient and family education on learning needs and treatment options.
• Initiates emergency response measures and participates in staff training as assigned.
